I used to occasionally do reception for a local mental health clinic -really hard to get an appointment. One morning we had 9 appts and not one person turned up. It is so frustrating

There is no expectation of accountability for patients/parents--there should be consequences for poor decisions.
as much as this frustrates you, Scrabbler, I will bet that more patients have been made to wait for hours after their appointments because the doctor is running late, so my suggestion is...........
to have a big sign in the waiting room that says, "the doctor is not able to see you on time because (fill in the number) patients did not come on time and demanded to be seen when they arrived.

I think it is simply a matter of telling a patient who is an hour late that the missed their time slot and can wait until everyone else has been seen, reschedule or see another doctor.
